Social Worker

Subdepartment of Health Department.
Department of Mental Health Elder Affairs

The Community Social Workers provide comprehensive professional social services to town residents of all ages. Services include case management, consultation, information and referral, advocacy and support, and crisis intervention.

The Community Social Workers accept referrals from residents (or concerned family members), schools, police and fire departments, clergy, hospitals, and area mental health agencies. They often work in collaboration with other providers and offer a flexibility to meet with clients in a variety of settings (e.g. home, school).

The Community Social Workers provide fuel and food assistance in conjunction with local/state/federal programs. They assist with the application process for subsidized programs such as housing and health care. They also act as a screening point for a number of financial assistance programs (such as the Community Assistance Fund.)

The Community Social Workers oversee contractual agreements with local therapists to ensure that all residents can receive outpatient mental health treatment (regardless of income). They also ensure casework services are provided for any frail or “at risk” seniors.
